CUSTOMER CONSENT AND
ACKNOWLEDGMENT AGREEMENT
By entering and using the facilities at Playdium, you, the customer, agree to the following terms and conditions, as well as provide consent for the activities outlined below. This document is intended to protect both the customer and Playdium and ensure a safe and enjoyable experience for all.
1. Waiver and Release of Liability
I acknowledge and accept the inherent risks associated with the activities and attractions at Playdium, including but not limited to arcade games, bumper cars, bowling, and other entertainment offerings. I agree to release Playdium and its affiliates from any liability for injury, loss, or damage to property that may occur as a result of participation in these activities.
2. Terms and Conditions of Service
I agree to abide by all rules and regulations set forth by Playdium, including those concerning conduct, safety, and behavior. I understand that violations of these rules may result in removal from the premises or denial of service without a refund.
3. Photo/Video Release Consent
I hereby consent to the use of my image, likeness, and voice in photographs, video recordings, and other media captured during my visit to Playdium. I understand that these materials may be used for marketing, promotional, or social media purposes and waive any rights to compensation for such use.
4. Data Collection and Usage Consent
I acknowledge that Playdium collects and stores personal data as part of its services, such as for loyalty programs, event registration, or promotional communications. I consent to Playdium’s use of my data in accordance with its privacy policy and for the purpose of improving customer experience.
5. Health and Safety Declaration
I confirm that I do not have any medical conditions, physical limitations, or impairments that would prevent me from safely participating in Playdium’s activities. Should I experience any health issues during my visit, I will notify a staff member immediately.
6. Parental/Guardian Consent (for Minors)
I, as the parent or legal guardian of a minor, provide my consent for the minor to participate in activities at Playdium. I acknowledge that I am responsible for the safety and well-being of the minor while on the premises.
7. Intellectual Property Acknowledgment
I recognize that Playdium holds the intellectual property rights to its games, equipment, logos, and other branded materials. I agree not to infringe upon these rights during my visit.
By entering Playdium, I confirm that I have read, understood, and agreed to the above terms. I acknowledge that my participation in any activities at Playdium is voluntary, and I accept the conditions set forth in this agreement.
